We use this questionnaire to record exactly what categories of information you want summarized and what information you'd rather not see. Completing the questionnaire will make your summaries more efficient for you and save you time in the future.

You can complete and submit the questionnaire by email, phone, fax, or postal mail.

In cases where no preferences are expressed we simply use our defaults.
